If you live within our practice boundary and wish to register with the practice, you will need to complete a registration form (PRF1). This can be collected from reception or you can download and print a copy.
Once completed please bring it to the surgery along with some photo ID and something with your current address on. (If you do not have these please discuss with a receptionist).
As a new patient you may be asked to attend for an introductory medical examination. This is an important consultation that allows us to discuss your past and present medical history and will ensure that you are receiving the best medical care possible.
Failure to attend your pre-registration health check, without good reason, may result in you not being accepted onto our list, and if you fail to attend two health check appointments you will not be accepted under any circumstances.
